I have never tried via ActiveX...

We use DLL wrappers with EZTwain Pro v3 which works very well.  The following is an
example of the code that we use to create PDFs of the scans:

STATIC PROCEDURE Acquire_And_Save(nScanType,nResolution,nQuality,cFile,lWithUI,oAppWindow)

     LOCAL hdib

     if eztw32_OpenDefaultSource()
        eztw32_SetHideUI(!lWithUI)
        eztw32_SetCurrentUnits(TWUN_INCHES)
        eztw32_SetCurrentResolution(nResolution)
        eztw32_SetJpegQuality(nQuality)
        eztw32_SetPixelType(nScanType)
        hdib := eztw32_Acquire(oAppWindow:getHWND())
        if hdib > 0
           eztw32_DIB_WriteToPdf(hdib,cFile)
           eztw32_DIB_Free(hdib)
        endif
        eztw32_CloseSource()
     endif
RETURN

You should be able to find more info about the EZTwain wrappers in this board somewhere.
